 FLU IMMUNIZATION REMINDER 

Choctaw Health Services will be at Calera School to provide flu immunizations.

 Thursday, October 1st
 They will be at Calera School by 9:30 AM.

A completed flu consent form signed by a parent or guardian is required in order for your child to receive the flu immunization.

 Consent Forms:

• Elementary students: Forms will be sent home with students on Tuesday, September 29th.
• High school students: Forms will need to be picked up from the High School Office.
